Cancellation for Next Round - Guests

Instructions:

• Click in the box for each player - type a few letters of the LAST NAME to jump in the list.
• Cancellations after noon two days prior to the round may be subject to a cancellation penalty.
• If you are cancelling more than four players, submit another form.
• If you would like to cancel a guest who is neither a member nor on the member waiting list, please contact the Tournament Chairman directly by phone or e-mail.
